Disorganization of the leadership structure breaks communication channel and flow of information and leads to misuse, abuse, and neglect, which breaks down the smooth operation hence attracting failure. Types of errors that exist are the bounded, imperfect and reluctant rationality errors, which all are either indivi dual or collective error in engineering. The licensing of engineering work in any organization, institution and level of work is crucial (Delatte 1-3).
